Asthma Education Gift Card

Asthma Education Gift Card

Texas Children’s Health Plan rewards you for taking control of your asthma! When you don’t have any symptoms, you won’t cough or wheeze, you can run and play as you want, you won’t have asthma attacks, and you won’t have to go to the emergency room. When you complete a series of 6 (six) asthma education classes over the course of 3 (three) sessions, we’ll send you a $50 gift card.

How do I claim my extra benefit?

You can register for this class by calling 832-828-1005, or visit the Texas Children’s Health Plan web site.   

How will I receive it?

Upon registration, you will receive a confirmation email with the time and location of the class. All classes will be held at The Center for Children and Women, either Greenspoint or Southwest locations.

Who is eligible?

This class is available at no cost to members ages 6 to 18 with an asthma diagnosis who are in the STAR, CHIP, or STAR Kids programs. 

Is this extra benefit available in my service area?

Class is available to members within the Harris Service area.

How do I claim my extra benefit?

You will receive a post card that you will need to fill out. You will then need to ask your service coordinator to verify enrollment in case management.

How will I receive it?

You will receive your extra benefit via U.S. Mail or you will have the option to receive your extra benefit in person during your case manager visit.

Who is eligible?

All members diagnosed with asthma and participating in Case Management who are in the STAR, CHIP, or STAR Kids programs.

Is this extra benefit available in my service area?

This extra benefit is available to members within all service areas, including Harris, Jefferson, and Northeast.


Post Hospitalization Gift Card

Post Hospitalization Follow-up Gift Card

Research has shown that good planning and a follow-up doctor’s visit can improve your health and reduce the chance of readmission. We encourage you to bring your child to see their doctor within 14 days of your child’s release from the hospital. We’ll send you a $20 gift card after your child completes this visit!

How do I claim my extra benefit?

You will receive a post card that you’ll need to take to your doctor. At the end of your child’s post hospitalization follow-up visit, fill out the card and ask your  doctor to sign verifying your child’s visit. Then, mail the post card, at no cost to you, to Texas Children’s Health Plan. You’re eligible to receive this extra benefit up to 2 (two) times per year. You may also request this post card by contacting your Service Care Coordinator directly or by clicking here.

How will I receive it?

You will receive your extra benefit via U.S. Mail.

Diabetes Gift Card

Take steps to manage your diabetes and get rewarded for improving your health! Receive a $20 gift card when you complete your Annual Diabetic Eye Exam, complete your Biannual HbA1c Blood Test, or Maintain a HbA1c blood result under 8 (up to $60 total).

How do I claim my extra benefit?

You will receive a post card that you’ll need to take to your doctor. The post card will have three sections, one for each exam completed. Fill out the card and ask your doctor to sign verifying you have completed the appropriate section. Then mail the post card, in a stamped envelope, to Texas Children’s Health Plan. Members are eligible to receive this extra benefit 1  (one) time per year for a total of $60 in rewards, $20 per exam completed.

You will receive your extra benefit via U.S. Mail. 

Who is eligible?

This extra benefit is available to our members 18 years of age or older and participating in Texas Children’s Health Plan Case Management.

Cervical Cancer Screening Gift Card

Every woman age 21 and older should get a cervical cancer screening (pap smear) during her well-woman exam. We’ll send you a $20 gift card for completing yours! 

How do I claim my extra benefit?

You will receive a post card that you’ll need to take to your doctor.  Ask your doctor to complete the post card once the well-woman exam/pap smear is completed. Then mail the post card, at no cost to you, to Texas Children’s Health Plan.

How will I receive it?

You will receive your extra benefit via U.S. Mail. 

Who is eligible?

This extra benefit is available to our members who are not pregnant, ages 21 to 64, and are in the Texas STAR program.

Is this extra benefit  available in my service area?

This extra benefit is available to members within Harris and Jefferson service areas.


Smoking Cessation Benefit

Texas Children’s Health Plan will help the parents of members under 20 years of age with up to $75 towards smoking cessation products.   

How do I claim my extra benefit?

Parents of Texas Children’s Health Plan members looking to claim this extra benefit will need to contact their Service Care Coordinator, Case Manager, or have received a doctor’s referral. 

How will I receive it?

Once your child’s eligibility has been verified with Texas Children’s Health Plan, you will be able to pick up your smoking cessation products from your local pharmacy. 

Who is eligible?

This extra benefit is available to parents of members who are 20 years of age or younger and who are tobacco dependent and have agreed to participate in a Smoking Cessation Coaching program.  Members should be enrolled in the Texas Children’s Health Plan STAR, CHIP, or STAR Kids programs.

24 Hour Nurse Help Line

You can talk to a nurse about your health concerns 24 hours a day, 7 days a week. Just call Texas Children’s Health Plan’s Nurse Help Line toll-free at 1-800-686-3831. Be ready to give your name, member ID number, and your symptoms. Texas Children’s Health Plan’s Nurse Help Line nurses cannot diagnose problems or recommend specific treatments. They can give you health information or tell you where you should go to get health services. They can also tell you how to take care of yourself at home when you don’t feel well, but don’t need to see a doctor. This is not an emergency line or a substitute for your doctor’s care.
